What are my rights & responsibilities as a patient? What are the possible causes of my symptoms (Note: This is also known as differential diagnosis)? How do you diagnose my condition? What are the different treatment options for my condition? What outcome should I expect from each treatment option?
Is there anything I can do on my own to improve my condition? What are the known side effects of the medication for my condition? How much will this cost me? Do you have experience managing this condition? Do you have a provider you can recommend for a second opinion? What questions haven’t I asked that I should have?
